Cochlear
implantation is a surgical procedure where an
electronic device is implanted behind the ear
with an electrode (wire with contact points)
is inserted into the cochlea, to bypass the
damaged “hair cells” which is believed
to cause hearing loss. Cochlear implantation
surgery, now performed worldwide in over a 100,000
people has evolved to become one of the safest
surgeries ever performed. At Dr. Manoj’s
ENT Institute, over 112 surgeries have been
performed, with new technological advances like
the “D” well, which protects the
implants against shocks and falls that young
children are susceptible to. We also pioneered
the minimal access surgery in this part of the

Subtotal
petrosectomy or Blind sac closure is a surgery
done for discharging ears with profound hearing
loss, or for cancer of the middle ear. In this
surgery, all structures and the mucosal lining
of the middle ear is removed and the ear canal
is sewed shut as a “blind sac”.
This gives an immediate dry ear and prepares
the ear for a possible cochlear implant later.

tympanicum is a tumor of the middle ear and
if found out early, can be removed without disturbing
the hearing mechanism of the ear and without
injury to the vital structures nearby like the
nerve to the muscles of the face. Through the
posterior tympanotomy, which is a very skilful
delicate procedure the tumor removal is demonstrated.
The patient can leave hospital the same day.

Following the principles of Phonomicrosurgery
and hydro dissection, using the most advanced
operating video laryngoscopes, vocal cord cysts,
polyps and small tumors are removed with minimal
trauma to the vocal cords. Patients feel no
pain, require almost no post operative medication
and are back to work the next day. Other phonomicrosurgical
procedures performed regularly include fat medialisation
for vocal cord palsy and Kashima’s operation
for breathing difficulty with paralyzed cords.
Ossiculoplasty
The
middle compartment of the ear has got three
small bones- the malleus, incus and stapes.
These bones are the smallest in the body and
are arranged in the form of a chain. They are
so important because it is only when they move
we have a good hearing. They can become fixed
or loose their continuity by infection, surgery,
temporal bone fracture or by birth. In order
to restore the hearing this chain has to be
reconstructed by a procedure called ossiculoplasty.
This can be achieved by with the help of bone
or cartilage. Polymers, ceramic and metal can
also be used.
Stapedotomy
This
is a surgery done for Otosclerosis. Otosclerosis
is a condition which affects mainly the stapes
and causes it to be fixed, thus causing a reduced
hearing. The condition can run in families and
corrected by stapedotomy. In this surgery, the
fixed stapes is replaced by a Teflon piston.
This is done under local anesthesia and the
patient goes home the same day.

Tympanoplasty
is a surgery for a ear which has been discharging
for a very long time with accompanying hearing
loss. In this procedure, the ear is opened through
a incision behind the pinna and the mastoid
and all the air cells within are opened up and
disease cleared, the hole in the diaphragm closed
with fascia and the diseased middle ear bones
are reconstructed. The patient goes home the
same day. Following the surgery, the patient
need not take bed rest. He can eat a normal
diet and travel.

Functional
Endoscopic Sinus Surgery (FESS) is a surgery
done mainly done for a chronic sinus disease
like sinonasal polyposis, allergic fungal sinusitis,etc.
The surgery make use of an endoscope and no
external incision is made. The Endoscopic Surgery
can also be applied to repair a CSF leak, removal
nasal and sinonasal tumours, orbital surgeries,
pituitary surgeries, infections of the lacrimal
sac,etc.
